Introduction
Your participation in the survey is voluntary and you may stop taking the survey at any time. Participating in this survey will be taken as your consent to participate in the housing needs assessment. This survey will take approximately five minutes to complete.
Confidentiality
The responses you provide will remain confidential and your name will not be collected as a part of this survey. All of your responses will be held and kept confidential. Your eligibility for services or assistance cannot and will not be impacted by your answers or by your refusal to participate.
The data from this research will only be reported as a collective, combined total, and no one will have access to your individual information.
Incentives
To show our appreciation for your participation in the community assessment, members of this household will receive a $5 gift certificate to the Red Cliff Fish Company for the purchase of food or apparel upon completion of the survey.
